What Causes Dupuytren's Contracture and Who Is at Risk?

Dupuytren's contracture is a progressive condition caused by thickening and scarring of the fascia beneath the skin of the palm. That thickened tissue forms cords that tighten over time, gradually pulling the fingers, most commonly the ring and small fingers, into flexion. The condition tends to run in families, is more common in men, and is associated with northern European ancestry, diabetes, and alcohol use, though it can occur without any of these risk factors.

The condition itself is not painful for most patients, but the functional limitation it creates is significant and worsens without treatment.

When Should You Seek Treatment for Finger Contracture?

Not every patient with Dupuytren's contracture needs immediate treatment. Small nodules without significant finger contracture can often be monitored over time. The threshold for intervention is generally when the contracture reaches approximately 30 degrees or more, or when the functional limitation begins to affect daily activities in a meaningful way.

A hand specialist can perform a tabletop test during evaluation: if the patient cannot lay their hand flat on a table, the contracture is typically significant enough to consider treatment.

How Xiaflex® Treats Dupuytren's Contracture Without Surgery

Xiaflex® (collagenase clostridium histolyticum) is an injectable enzyme that breaks down the collagen-rich cords causing the contracture. The treatment involves two office visits spaced approximately 48 hours apart. On the first visit, the enzyme is injected directly into the cord. On the second visit, the physician manipulates the finger to rupture the weakened cord and restore extension.

There is no incision, no general anesthesia, and no operating room. Recovery is measured in days to weeks rather than the months that surgical fasciectomy can require. Most patients return to light activity quickly and resume normal hand use within a few weeks of treatment.
